Not only do your comments help us with our continuous improvement efforts, they also ensure that we are in compliance with the CPSO IHF Clinical Practice Parameters & Facility Standards for Diagnostic Imaging.     Please rate the following on a scale of 1 to 5:
    1: Does not meet Expectations / 5: Exceeds Expectations

    1. Appointment wait times are within reasonable limits
    2. Urgent and STAT cases can be accommodated as needed
    3. I receive routine reports back within an acceptable amount of time
    4. Reports are concise and comprehensive
    5. When necessary, I am able to consult with Radiologists
    6. STAT, urgent and verbal report requests are provided in a timely manner
    7. I am informed of any unsuspected or critical findings in an appropriate way.
    8. The requisition form is clear and easy to use

    From time to time, our Radiologists or staff may want to contact your office to discuss a patient's history, exam indications or share with you a critical or urgent finding. Could you please provide/confirm a direct phone line that we may use for this purpose?
